New enhanced Standard device design

The Discluding Element (DE) has been enhanced from the orignial
design (left) being extended both distally and anteriorly (right) to
accomodate for Class II occlusions and extreme protrusive
movements (avoids having to add acylic to the DE, as would be
necessary when using the original design).
